Cost types per cost line, and a gebrek wizard that fits the vlak

This release gives per-urgentie cost reports a type per line, and narrows the gebrek choices on the phone to what fits the vlak.
Kostentype per cost line
On a report per urgentie, you set the type per cost line in the Type column. Choose "Geen type" to clear it again. The type is also printed in the PDF report and in the shared report link. NHG reports and yearly reports do not show the column.
Only the gebreken that fit the vlak
On the phone, the gebrek wizard offers only the gebrek soorten and belemmeringen that fit the vlak you are inspecting. You pick from a shorter list, and the options that do not belong to that vlak stay out of the way.
